Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. On or about May 6, 2024 located at 1755 Hillard Rome Road, Hilliard, Ohio, 43026: Employees were exposed to struck-by hazards associated with moving/falling parts while performing service and maintenance work on a Caterpillar 349F excavator. The employer failed to implement an energy control procedure to manage the hazardous energy associated with this equipment. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)] and verification of isolation [(d)(6)], were not implemented to protect employees.
