29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval: On or about July 5, 2023, employees of Miguel Gonzalez utilized a Star Industries manufactured trash hopper with a Genie GTH-1056 Telehandler without receiving the manufacturer's written approval, exposing employees to struck-by and tipping hazards.
