29 CFR 1910.265(c)(3)(iv): Floors, footwalks, and passageways in the work area around machines or other places where person(s) were required to stand or walk were not provided with effective means to minimize slipping and or tripping: a) At the tree cut saw or or about 2/14/2022: Area around the cutup saw was not maintained, in that excess log bark, wood dust and ice created slip and tripping hazards which resulted in a work-related amputation as the worker slipped onto a chain and sprocket.
