29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146: (a) On or about 7/16/2021, and at times prior to - 4440 Chisolm Road Florence, AL 35630, Production Department, employees accessing the Hunter 20 pit, and other confined spaces containing potential atmospheric, engulfment, machine, or other recognized hazards were exposed to caught by, caught between, and/or struck by hazards from unguarded machines because the employer did not develop and implement a written PRCS program to instruct employees of the dangers posed by the permit spaces.
