Note: If such modifications or changes are made, the capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly: On or about July 14, 2021, and at times prior thereto, a forklift attachment was used as a working platform to lift personnel without the forklift manufacturer's written approval and decals, tags, and plates changed, exposing employees to fall and struck-by hazards.
