29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Plant Area - On or about June 2, 2021, the employer failed to provide effective guarding to protect employees working at or near the Briquetter Machines #3. An employee's foot became crushed after being struck by an hydraulic door during operation. The guarding device for this machine, Briquetter Machines #1, did not prevent employees from been in the zone of danger during the crushing process. Employees were, thereby, exposed to a struck-by and amputation hazards. b) Plant Area - On or about June 2, 2021, the employer failed to provide effective guarding to protect employees working at or near the Briquetter Machines #2. The guarding device for this machine, Briquetter Machines #2, did not prevent employees from operating the machine while in the zone of danger during the crushing process. Employees were, thereby, exposed to a struck-by and amputation hazards. c) Plant Area - On or about June 2, 2021, the employer failed to provide effective guarding to protect employees working at or near the Briquetter Machines #3.
