29 CFR 1928.57(a)(6)(iii):Instruction was not provided to stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment: The employer is failing to protect employees from rotating machinery components when unclogging grain vacuums. This was most recently documented on May 25, 2021 at 2609 190th St, Albion, Nebraska when a worker was attempting to manually unclog a grain vacuum that had residual energy resulting in the amputation of two fingers. The worker had not received instruction to wait for all machine movement to stop before attempting to unclog the vacuum.
