29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about May 19, 2021 - Drilling station in plant, employees were exposed to struck-by hazards when the back of the machine was not guarded and drill bits/hold down bolts could be inadvertently accessed while the machine was running. (b) On or about May 12, 2021 - Drilling station in plant, employees were exposed to struck-by hazards when it was necessary to place boards onto the bed of the machine while the drill bits were turning. (c) On or about May 12, 2021 - Drilling station in plant, employees were exposed to struck-by hazards when it was necessary to hold boards onto the bed of the machine while causing the bed to rise toward the turning drill bits and the spring loaded hold down bolts.
