29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about November 30, 2020, at the Midwest Scrap Management yard located at 850 East 45th Street North Park City, Kansas, the employer is failing to protect employees from struck-by and crushing hazards. The employer failed to conduct and document a periodic review of the energy control procedures for the yard equipment exposing employees to struck-by and crushing hazards while performing servicing and maintenance activities including clearing jams. Equipment including but not limited to the Harris HS 98115 auto shredder machine, the balers, the crane-cable, the material handler crane, the forklift, the skid steer loaders, the hydraulic shears, and the truck. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is complete.
