29 CFR 1926.1203(a): Before work began, the employer did not ensure that a competent person identified all confined spaces in which one or more of the employees are to enter and evaluated any permit required confined spaces. On or about October 13, 2020, and at times prior thereto, employees working in a sewer line manhole, a permit required confined space, were exposed to hazards such as, but not limited to, flammable gases, oxygen deficient atmosphere, or toxic air contaminants.
