29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: A.) George Junior Republic dba George Junior Republic in Pennsylvania, 233 George Junior Road, Grove City, Pennsylvania 16127: An employee was hospitalized following a workplace injury on August 12, 2020. The hospitalization was not reported to the OSHA Erie Area Office within 24-hours of the hospitalization. The condition was noted on or about August 19, 2020. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
