29 CFR 1926.451(b)(4): Each end of a platform, not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches (15 cm): On or about December 19, 2019, adjacent to an elevator inside a concrete poured residential structure under construction located at 1005 NE Doubloon Drive in Stuart, Florida, employees were exposed to a fall hazard of approximately 8-feet, 11-inches when performing glass installation work on a fabricated platform scaffold where the work platform did not extend at least 6-inches past the centerline of the support beam.
