Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Naval OLF NEX: On April 23, 2019, employee(s) were repairing a wireless pressurized line leak detector (WPLLD) in a fuel sump while the equipment was energized. The WPLLD broke and fuel sprayed from the equipment. Employee(s) manually disengaged the yolk assembly to isolate the energy, which ignited the fuel vapors, and created a flash fire. De-energization of the fuel pump was not achieved prior to performing maintenance on the fuel sump system. Employees were exposed to fire and/or explosion hazards.
