29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Mid-States Packaging, Inc., located at 12163 State Route 274, Lewistown, OH 43333: On or about April 15, 2019, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, were performed. Employees did not deenergize the 100 Ton Freeman Schwabe Automatic Press (Model # BP100-5979H; Serial # FHH1009) prior to conducting activities such as, but not limited to, attempting to find air leaks. Employees were exposed to the hazards that could result in personal injuries while all hazardous energy sources were not controlled prior to performing the tasks.
