29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Holtville Gin Co-op; On or about February 14, 2018 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were required to work on equipment to include but not limited to the cotton gin without first affixing a lockout device on the machines isolating device to secure the machine from the energy source.
