29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: On or about July 5, 2023, and times prior thereto, work platforms occupied by employee(s) were moved by a telehandler horizontally while occupied by employees, thus exposing the employee(s) to fall and struck-by hazards, in the following instances (a-b): a) Worker sitting on a pallet being lifted to the roof by the telehandler; and b) Two (2) workers occupied the Star Industries manufactured trash hopper being lifted to the roof by the telehandler operator.
