29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: a. On or about July 26, 2019, at the above addressed jobsite, employees was required to work inside sump pit, a permit-required confined space. The employer did not ensure to document the completion of measures required by the confined space program. The employer did not prepare entry permits, test the atmosphere in the space and verify that the requirements in paragraph (d)(3) had been completed prior to entry, thereby exposing an employee to hazards associated with confined space entry.
