Text For Citation: 01 Item/Group: 002B Hazard:
29 CFR 1910.132(f)(2): The employer did not require employees to demonstrate an understanding of the training specified in paragraph (f)(1) of this section, and the ability to use personal protective equipment (PPE) properly, before being allowed to perform work requiring the use of PPE: On or about July 3, 2014 and in times prior thereto a Miller derrick belt was in use and the user instructions were not provided. The derrick man was not trained as to the hazards of hooking off to the tool rings that were not designed for fall protection. The waist belt was for work positioning only and the rings were not intended for fall protection. The D ring in the upper back section of harness was the only ring designed for fall protection and the employer did not ensure it was used. The fall hazard was located in the derrick of Drilling Rig #4.
