29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. a. On or about June 22, 2023, at the jobsite located at 6084 Alligator Lake Shore West, Saint Cloud, Florida, employees were exposed to a crush-by hazard while constructing a retention wall, in that, the employer failed to have a competent person inspect the adjacent areas such as a concrete column when the soil and adjacent support system were removed.
