29 CFR 1910.178(l)(4)(ii)(A): Refresher training in relevant topics was not provided to the operator when the operator had been observed to operate the vehicle in an unsafe manner: (a) On or about June 17, 2020 - 2701 Southeastern Circle, Birmingham, AL, employees were exposed to struck-by hazards when forklift drivers who had been observed operating forklifts in an unsafe manner such as, but not limited to, driving without looking in the direction of travel and driving without a seatbelt did not receive refresher training.
