29 CFR 1926.352(e): When the welding, cutting or heating operation was such that normal fire prevention precautions were not sufficient, additional personnel were not assigned to guard against fire while the actual welding, cutting, or heating operation was being performed, This violation occurred on or about May 19, 2020 and at times thereafter, on the Pelican Island Storage Terminal located in Galveston, Texas, when fire watcher was not assigned in the work area where employee was welding pipe support pads onto the roof of storage tank 106-37 which contained flammable chemicals such as Low Sulfur Vacuum Gas Oil. This condition exposes workers to the hazards of a catastrophic equipment failure including being struck by a high energy release, equipment parts, and fire/explosion.
