29 CFR 1910.23(d)(4): The side rails of through or sidestep ladders did not extend 42 inches (1.1 m) above the top of the access level or landing platform served by the ladder: (a) On or about March 9, 2020, and at times prior, employees were exposed to a fall hazard while accessing the second level storage area in the northeast corner of the shop by way of the fixed ladder. The rails at the top of the ladder extended only inches above the landing platform.
