29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): (a) On or about 1/16/2019 - Unit 2 Pulverizer 2C, the employer did not conduct an assessment of the workplace after changing from the old style pyrite box system to the new style pyrite box system to determine if new hazards were present that would require additional PPE.
