Text For Citation: 01 Item/Group: 002A Hazard:
29 CFR 1910.132(f)(1)(iii): Employee(s) required to use personal protective equipment (PPE) by this section were not trained to know how to don, doff, adjust, and wear PPE: On or about July 3, 2014 and in times prior thereto a Miller derrick belt was in use and the user instructions were not provided. The derrick man was not trained as to the hazards of hooking off to the tool rings that were not designed for fall protection. The waist belt was for work positioning only and the rings were not intended for fall protection. The D ring in the upper back section of harness was the only ring designed for fall protection. The hazard was located in the derrick of Drilling Rig #4.
