29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected: On or about January 6, 2025, at the worksite located at 1131 Dakota Avenue, Dakota City, Nebraska, employees engaged in maintenance activities were exposed to caught-in and crushing hazards. The employer did not ensure periodic review for the Newcastle Load Transfer machine, serial number 14134, included a review of the procedure with all employees authorized to lockout/tagout the machine.
