29 CFR 1926.307(d)(1): Pulley(s) with part(s) seven feet (2.128 m) or less from the floor or work platform were not guarded in accordance with the requirements specified in paragraphs (m) and (o) of this section:Note: Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than 6 ft. 6 in. (1.976 m) from the floor or platform may be guarded with a disk covering the spokes. At the jobsite located at the bridge construction over the Arkansas River in Nickerson, KS, the crane motor located inside the crane cab had an unguarded pulley, exposing employees to caught-in and amputation hazards. Employees engaged in crane activities as a part of their daily duties were exposed to caught-in and amputation hazards in that the employer did not provide a guard for the pulley located inside the cab of the American Crane.
