29 CFR 1910.178(l)(2)(i): Trainee(s) were operating powered industrial truck(s) without direct supervision and/or when there was a potential to endanger themselves or other employees as required: (a) On or about June 17, 2020 - 2701 Southeastern Circle, Birmingham, AL, employees were exposed to struck-by hazards when a trainee who had been certified to drive a forklift on June 17, 2020, drove backwards without looking in the direction of travel on June 17, 2020 and forked a drum on June 16, 2020.
