Text For Citation: 01 Item/Group: 002A Hazard:
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A.) Microtech Knives, Inc., 300 Chestnut Street Ext., Bradford, PA 16701, on or about November 22, 2019 and times prior thereto: Employees clean the magnet table of the Okamoto ACC-16-32DXV surface grinder (S/N: 68566) after knives are removed from the table. During the cleaning process, the surface grinding wheel is still running. On or about November 22, 2019, an employee sustained an amputation from the moving blade while cleaning the magnet table. The surface grinder was not powered off, and locked out to prevent employees coming into contact with the hazardous rotational energy of the grinding wheel. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
