29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was isolated from the energy source and rendered inoperative: Instances including, but not limited to, the following: a) Job site, 607 Penn Avenue, Pittsburgh, PA: On or about October 1, 2017, during work activities on HVAC systems in the building, the employer did not ensure that an appropriate energy control program was in place in that an employee performed worked that required energy control but did not receive energy control training, written energy control procedures did not exist and locks and other hardware needed to control energy were not provided.
