29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: (a) Vehicle Maintenance Building: The left side mezzanine in the parts department had chains used in lieu of fixed guard rails where material was hoisted and the chains were not at the minimum allowed height to protect employees from falling approximately nine feet in that the middle of the top chain sagged down to twenty-nine inches and the mid chain sagged in the middle down to eighteen inches as observed on June 11, 2013. (b) Vehicle Maintenance Building: The right side mezzanine in the parts department had chains used in lieu of fixed guard rails where material was hoisted and the chains were not at the minimum allowed height to protect employees from falling approximately nine feet in that the middle of the top chain sagged down to twenty-nine inches and the mid chain sagged in the middle down to fifteen inches as observed on June 11, 2013. (c) Track Maintenance Building: The mezzanine had chains used in lieu of fixed guard rails where material was hoisted and the chains were not at the minimum allowed height to protect employees from falling approximately nine feet in that the middle of the top chain sagged down to thirty-one inches and the mid chain sagged in the middle down to eighteen inches as observed on June 11, 2013. ABATEMENT VERIFICATION REQUIRED.
