29 CFR 1926.21(b)(6)(i): Employees required to enter into confined or enclosed spaces were not instructed as to the nature of the hazards involved, the necessary precautions to be taken, and in the use of protective and emergency equipment required: The employer does not instruct employees required to enter confined or enclosed spaces on the hazards involved in confined space entry, the necessary precautions to be taken, and on the use of protective and emergency equipment. This violation occurred on or about September 26, 2013, at pump station #1 of the Eastside Water Treatment Plant located at 405 Long Creek Road in Sunnyvale, Texas, where two employees, who entered a dewatering pit, had not been instructed on the hazards involved in confined space entry and the necessary precautions to be taken.
