Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: Well Site near Kelton, TX, Rig #134, Well: Neeley #505: On 7/29/08, and times prior thereto, employees were exposed to potential struck by hazards while they were performing job tasks on the rig floor as mud was circulating under high pressure through circulating pipe which was connected by a hammer union of mismatched/unlike halves. American Petroleum Institute Recommended Practice RP54 Paragraph 9.13.5 states that: "Hammer unions shall be made up of like halves with the same pressure ratings and thread type. Many connecting threads look alike, but will fail under working conditions." Note: Abatement certification and supporting documentation are required for this item.
