29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. a) On or about December 4, 2024, at the SPA Center - Studio A, closet 1, employees performing air filter change were exposed to 9.5-feet fall hazard, in that, the access hole to the fully decked attic was not protected by a cover or a guardrail system. b) On or about December 4, 2024, at the SPA Center - Studio A, closet 2, employees performing air filter change were exposed to 9.5-feet fall hazard, in that, the access hole to the fully decked attic was not protected by a cover or a guardrail system.
