29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Sawmill Chipper area - Slant Disc Chipper: On or about February 5, 2024, and at times prior, the employer exposed employees to amputation and electrical shock hazards in that, the employer failed to ensure that machine specific procedures were documented for the control of potentially hazardous energy when employees performed service and maintenance of equipment including, but not limited to, ACROWOOD Slant Disc Chipper (Model: 8424-6K, S/N: 0170).
