29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems that were used: A). On or about 10/03/2023, at the site of 140-36 Negundo Ave. Flushing, NY Employees utilizing a personal fall arrest system were not trained in the appropriate method of installing an anchor point. Only 3 out of 32 fasteners were used to secure the roof anchor bracket to the roof.
