29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): FEDERAL CORRECTIONAL INSTITUTION - PHOENIX: The Internal Revenue Service did not assess the firing range or live fire operations to determine if hazards were present which necessitated the use of personal protective equipment such as ballistic vests, safety glasses, or hearing protection. On August 17, 2023, an IRS Special Agent was fatally shot during a routine training exercise.
