Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: a) 405 South Reading Road Ephrata, PA - On or about May 25, 2023, employees were performing work on an aluminum dome roof with a height of between 18 feet and 22 feet above the concrete lower level, without their fall protection equipment, such as the D-Ring extension, positioning strap, and rope grabs, being inspected prior to use. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
