29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about May 14, 2023, at Prairie State Generating Company, Submerged Flight Conveyor #1, two employees were exposed to hazardous energy generated by the weight of the conveyor flights and chain applying pressure to a drive system. The employer failed to implement energy control application steps and the employees conducting the work did not have knowledge of type and magnitude of the gravity energy present [(d)(1)], did not isolate the dry side of the chain to prevent gravity energy to be applied to drive system [(d)(3)], did not relieve, disconnect, restrain, or render it safe [(d)(5)], and did not verify that the drive system was isolated.
