29 CFR 1910.146(c)(5)(ii)(C): Before an employee entered a space, the internal atmosphere was not tested with a calibrated direct-reading instrument for oxygen, flammable gases and vapors and potential toxic air contaminants: a) Pan mixer; On or about March 15, 2023, and at times prior to, the employer exposed employees to a potential oxygen deficient atmosphere and respirable dust hazards in that employees were required to work within a permit required confined space without first ensuring that a hazardous atmosphere did not exist within the space.
