29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): a. On Courtney Vista Drive in Saint Augustine, Florida: On or about July 21, 2022, the employer exposed two underground utility construction employees to a 8-foot cave-in hazard, in that the employees worked in areas outside of the trench box without a cave-in protection system.
