29 CFR 1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. A) On or about April 26, 2022 at 118-37 Metropolitan Avenue, Kew Gardens, NY 11415 An access ladder was not provided for an employee required to access a two (2) point suspended scaffold to perform exterior brick work on the 2nd story of a residential apartment building resulting in an employee falling approximately 20 feet to the lower level on April 26, 2022. ABATEMENT CERTIFICATION IS NOT REQUIRED
