29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of carbon monoxide listed in Table Z-1 in excess of the 8-hour Time Weighted Average concentration of 50 parts per million (ppm): The employer failed to ensure that employee exposure to airborne concentrations of carbon monoxide does not exceed 50 ppm, as an 8 hour time-weighted average (TWA). On or about February 17, 2022, at Hornady Manufacturing at 3625 W. Old Potash Hwy, Grand Island, NE 68803, an employee performing inventory in Building B-22 while utilizing a forklift was exposed to carbon monoxide during a 135-minute occupational exposure at a mean of 752.6 parts per million. The carboxyhemoglobin level in the employee's blood reached a calculated peak of 42.5%. The 8-hour Time Weighted Average (TWA) concentration of carbon monoxide was 211.7 parts per million CO, which is 4.23 times the OSHA-PEL of 50 ppm.
