29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) Ohio Polytech, LLC at 201 E. Beal Avenue, Bucyrus, OH: On or about February 24, 2022, the employer did not ensure that each employee tasked with operating the forklifts at the "Shredder Line" in the "Reclaim Area" received an evaluation of each operator's performance of safe operation and recertification every three years as required by paragraph (l) of this section, exposing employees with struck by hazards.
