Maintenance shop: The horn on the Toyota forklift used to move equipment was not operational. Employees were exposed to struck-by hazards. b: Maintenance shop: The rear left tire on the Toyota forklift used to move equipment had a large chunk of rubber broken off from the tire. Employees were exposed to struck-by and crushed-by hazards.
