29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section: On or before November 23, 2021, the company's lockout/tagout program was deficient in that specific procedural steps or additional methods and instructions to protected employees from unguarded moving parts and entanglement/entrapment hazards while they serviced the shavings bin that was equipped with rotating auger were not developed and their use enforced at the facility. On November 22, 2021, an employee tasked with fabricating a cover for the shavings bin and auger was fatally injured when he got pulled into the unguarded auger that was operational at the time of the incident.
