29 CFR 1910.305(b)(2)(i):Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: Employees were exposed to fire and electrical hazards. Employees engaged in material handling activities around the backroom refrigerator were exposed to fire and electrical shock hazards in that the outlet boxed did not have covers fully attached.
