29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about July 12, 2021 a laborer, was exposed to amputation hazards during his work shift due to an unguarded rebar bending machine, Schnell Model P55, Serial Number 0090040177. Utility Concrete Products, LLC did not ensure that point(s) of operation of machinery were guarded to prevent employees from having any part of their body in the danger zone during operation. Abatement documentation is required for this item in accordance with the requirement of 29 CFR 1903.19(d).
