29 CFR 1910.268(n)(2)(i): Dead-end poles were not tested in accordance with paragraph (n)(3) of this section and determined to be safe before employees were permitted to climb them: On or about May 24, 2021, employees were exposed to fall and crush/struck-by hazards when climbing Southern pine treated penta, wooden utility pole that had not been physically inspected, prior to climbing.
