A) C.H.A.T of Pennsylvania, On or about May 05, 2021 at the address of 5650 Wattsburg Rd., Erie, PA: Employees did not utilize protective eye equipment such as safety glasses and/or similar face protection during work tasks including, but not limited to dismantling of storage racks.
