29 CFR 1910.147(f)(3)(ii)(D): Group lockout or tagout devices were not used in accordance with the procedures required by paragraph (c)(4) of this section by requiring each authorized employee to affix a personal lockout or tagout device to the group lockout mechanism when he or she begins work, and to remove those devices when he or she stops working on the machine or equipment being serviced: a) at the facility, on or about January 23, 2021, where a group lockout or tagout device was not used when multiple employees performed maintenance and servicing activities on the Roundo PS-255 plate roller, serial #935425, exposing employees to amputation hazards.
